Below is a link to download the intake forms I ask new clients over the age of 16 to complete.  You need to download the file, open it with Adobe, fill out the forms online and email them back to me using the "SUBMIT FORMS" button on the last page. If you view the document in a preview pane, you will be unable to see the signature lines. Please open the file from your downloaded files list using Adobe Acrobat. 

 

 

 

 

Alternatively, you can completed forms online, print them, scan them and email them back that way.

Insurance Information:  

I currently participate with Anthem, Aetna, CIGNA, HUSKY, and plans administered by United Healthcare Behavioral Health (Optum). If you are not a member of any of these plans, I can provide you with an itemized receipt for services that you can submit to your insurance company so you can get reimbursed.  Please be sure that your specific plan will allow for out-of-network providers.
